  • Student's full name
  • All Symptoms
  • Date of onset of illness
  • Call back number
Student must have a negative PCR test before returning to campus (see Test Off-Campus for locations)

Testing Permission

AFTER you have signed up with Inspire Diagnostics, contact the office if you want your child to be tested weekly or for an exposure event only.


If you want your student to be included in surveillance and exposure event testing using PCR testing, please register with Inspire Diagnostics right away to avoid delay at testing time.

did you get an exposure notice?

If your child is symptomatic, please keep them home, notify school, and schedule a test. (see Test Off Campus for locations)
If your child is asymptomatic and:
  • Unvaccinated.. they can stay in school if they get tested at the next on-campus PCR testing date. (see Test On-Campus)
  • Vaccinated..they do not need to be quarantined, but you must show proof of vaccination.
